The cover depicts a man (blue jacket with black highlights, red tie swinging over his shoulder, yellow cap) driving a convertible (red body, yellow grill, green and yellow running boards and fenders over front wheels) along a narrow yellow path with a mountain (white and light blue with black stripes) behind and a sharp cliff in front. The right front tire (white and black) is rolling ahead of the car.

The title appears at upper right: "The TIRE" above "CAME OFF" in black sans serif text, though "The" is in a semi-script font.

The border of the cover consists of alternating squares with black borders: red, yellow, and blue, on all sides of the cover.
